The Appointment Process for New Psychiatric Patients
If you are interested in becoming a new patient at PSGP, you now have two options for completing our New Patient Intake Packet. You may fill out and submit the forms online using our secure digital link, or you can download and print the packet to complete by hand. Printed packets can be returned by fax, mail, or dropped off in person at our Roseville office. Please see the links below for your preferred method of submission.
