The Appointment Process for New Psychiatric Patients
If you are interested in becoming a new patient at PSGP, please download and complete the New Patient Intake Packet below. You can also use websites such as FormSwift or Adobe Fill & Sign to complete the forms and then download and send to our office via email or fax. Please be sure all forms are completed, and if any pages do not apply to you, please put "N/A." New patients will not be scheduled if all pages are not completed. Once you submit your packet via email or fax, someone from our Intake Department will be happy to contact you and set up your new patient evaluation.
